Your Primary Care Mental Health Service team provides local specialist mental health support for your surgery for anyone between 17-65 years of age. Working with GPs, the PCMHS offers early assessment, treatment and/or onward referral in the community. They support people with mental health challenges to access advice and support, focus on achievable goals and access local resources.
GPs can make an e-referral to the Primary Care Mental Health Service by completing a Request for Service form on eRS. GPs will ensure patients have given consent for the referral and SystmOne shares are in place to allow joined-up care. The PCMHS then offers assessment either face-to-face, via online video assessments (Attend Anywhere) or by telephone.
